Small Group Training

Work with others to achieve your fitness goals! Small Group Training courses are 6 week, progressive, small session fitness classes designed with a specific goal or objective in mind. Two 6 week (12 total classes) Small Group Training sessions are offered per semester. For each new 6 week period, a Small Group Training pass must be purchased to remain elligible to attend Small Group Training classes.

Physician's Referral Form

This form must be completed by physician and brought to first meeting with the personal trainer.

Registration and Medical Screening Form

This form must be brought into the Main Office with payment.

Males over 45 years of age or older, females 55 years of age or older or those who have one or more risk factors identified on this form are required to complete the Physician Referral Form.
